Hi. Rebased patch set on the current master. The only noticeable change
is that resetting of async requests moved from ExecAppendAsyncReset() to
ExecReScanAppendBase(). ExecAppendAsyncReset() now only drains them, and
so renamed to ExecAppendDrainAsyncRequests().
