Thanks very much for the SNI fixes and for reviewing the tests.

Attached is v2 with inline tests and an ordinary SKIP block for the
case that requires forked backends. It replaces my previous test patch
and still applies on top of v5. The three scenarios are unchanged, and
the added tests pass with Test::More 0.98.
