I also encountered the same issue one year before:

At that moment, I identified its cause:

At last it was fixed:
http://archives.postgresql.org/pgsql-hackers/2010-11/msg01910.php

But Steve encountered it again, which means that the above fix is not
sufficient. Unless the issue is derived from my patch, we should do
another cycle of diagnosis of it.
