And now v7, which fixes the bug in v6 that caused issues in the
integration suite, which I missed because I'd only tested with
regress+recovery.
The bug was that I failed to copy the visibility-related fields of the
heap tuple with inlined toast fields. Now that I've added that to the
patch the full test suite seems to pass just fine.
