Log Message:
Add new psuedo threshold type 'IGNORE' - queries whose cost is less than this
are not queued at all! This should make queueing work better with ad-hoc
tools that do a lot of catalog lookups before running actual queries.

Initdb is forced (changes to pg_resqueue).

Modified Files:
        resource-scheduler.sgml (r1.4 -> r1.5)
        alter_resource_queue.sgml (r1.6 -> r1.7)
        create_resource_queue.sgml (r1.4 -> r1.5)
        queue.c (r1.11 -> r1.12)
        gram.y (r1.5 -> r1.6)
        keywords.c (r1.4 -> r1.5)
        resqueue.c (r1.17 -> r1.18)
        resscheduler.c (r1.15 -> r1.16)
        catversion.h (r1.5 -> r1.6)
        pg_resqueue.h (r1.4 -> r1.5)
        resscheduler.h (r1.13 -> r1.14)
